BOOKING AND INTAKE REQUIREMENTS 1. IV therapy and acupuncture are clinical services delivered by licensed practitioners. An intake form is mandatory before your first appointment and must be completed at least 24 hours in advance. 2. Your intake form covers medical history, current medications, allergies, and conditions relevant to your treatment. For IV therapy, this includes information about kidney and heart conditions, blood disorders, current supplement use, and any known allergies to IV components. 3. IV therapy requires a brief consultation before your first session. This is done in person at the start of your appointment and takes approximately 10 minutes. Your drip formulation may be adjusted based on your consultation findings. 4. Acupuncture intake covers your primary concern, relevant health history, stress and sleep patterns, digestive health, and current medications. This information directly informs the points your acupuncturist selects — it is not routine paperwork. CANCELLATION & RESCHEDULING 1. We require 24 hours’ notice to cancel or reschedule an IV therapy or acupuncture appointment. 2. Cancellations with 24 or more hours’ notice: your credit is returned in full. 3. Cancellations with less than 24 hours’ notice: a 50% late cancellation fee applies. 4. No-shows are charged 100% of the service value. 5. If you are feeling unwell on the day of your appointment, please contact us. We will reschedule you without a fee. Receiving IV therapy or acupuncture while acutely unwell may not be appropriate, and we would rather see you when your body is ready to benefit fully. SAFETY & CONTRAINDICATIONS 1. Certain medical conditions may mean that IV therapy or acupuncture is not appropriate for you, or requires physician clearance before proceeding. These include but are not limited to: active infections, blood clotting disorders, kidney disease, congestive heart failure, active cancer treatment, pregnancy (for certain IV formulations and acupuncture points), and specific medication interactions. 2. If you are under the care of a physician for any ongoing condition, we encourage you to let your doctor know you are receiving IV therapy or acupuncture. We are happy to provide documentation of your treatments on request. Reset practitioners do not diagnose medical conditions. IV therapy and acupuncture at Reset are wellness services, not replacements for medical care.
